New York State Health Department Extends Contract With U.S. Care Management for Medicaid Care Management Program in Western New York
Jacksonville -

JACKSONVILLE - U.S. Care Management has announced that the New York State Health Department has extended its contract to provide community-based disease management services to help address the complex health care needs of select Medicaid recipients with chronic conditions or at risk for a disease in the seven-county area of Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans and Wyoming.

The program, launched last year, promotes the development and implementation of innovative approaches to providing disease and care management services for Medicaid fee-for-service recipients who have one or more of the following chronic illnesses: heart failure, chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD), asthma, diabetes, sickle cell disease and mental health conditions including adult depression, schizophrenia, schizoaffective and bipolar disorder.

U.S. Care Management, a wholly owned subsidiary of U.S. Preventive Medicine, manages a team of RN Care Managers and community outreach representatives who live and work in the seven-county region. Nurses provide face-to-face and telephonic assistance to recipients offering care coordination, education and monitoring services to improve self care, adherence to their doctor's treatment plans and understanding of available services and treatment options. A 24 hour, seven days a week toll free nurse phone line is also available for clients to access information.

Frederic S. Goldstein, president of U.S. Care Management, stated, "It has been our privilege to work with the New York State Health Department this past year and we are thrilled that they have expressed their confidence in our program and services by extending this contract. Our project in the Western New York region offers another opportunity for U.S. Care Management to use our community-based disease management model to contribute to the health of individuals with chronic health conditions. We look forward to continuing our work with those Medicaid recipients in the program, their health providers, community services and the New York State Health Department in achieving positive outcomes."

U.S. Care Management is based in Jacksonville, FL, and consistently demonstrates positive quality, cost, and satisfaction outcomes in the management of patients with complex chronic conditions. The company contracts with government and private payers of health services and operates in several states throughout the country.
